Blood Sent to NINDS Human Genetics Repository (at Coriell)

      Specimen Collection Form (Form 42)

      Investigators who submit data regarding a subject to the PD-DOC Database are required to request permission from the subject to have blood drawn and submitted to the NINDS Human Genetics Repository (at Coriell) as a permanent source of DNA for research studies.  This form indicates whether the subject consented to have blood sent to Coriell and whether or not it was collected.

Submitting blood samples 

The Coriell Cell Repositories in Camden, NJ, collects, stores and distributes medical research information, cell cultures and DNA samples to researchers in a masked fashion. White blood cells will be used to make a cell line and DNA. Cell lines allow Coriell to have a source of the DNA without having to redraw blood.

Sample Informed Consent Language

Your informed consent must contain language describing the collection and use of the blood samples for the the NINDS Human Genetics Repository (at Coriell). Once you have included the model consent language in your informed consent and prior to submitting to your IRB, your consent should be sent to the PD-DOC Program Manager who will forward it to NINDS to be reviewed. 

The NINDS Repository Investigator Approval and Tracking Data Form must be completed and sent to Coriell prior to submitting any samples.  Once completed Coriell will send the required number of blood sample NINDS Repository Investigator Approval and Tracking Data collection kits to the site.

Frequently asked questions

Clinical data elements. This clinical information must be submitted to Coriell along with the blood samples.

As a submitting investigator, you will receive 20 µg aliquot of DNA for each sample submitted, cell lines for a nominal fee, and have access to samples collected by others at a minimal cost. Every sample submitted must be accompanied with clinical data. All samples are shared after a grace period of no more than three years.
